Job Title: Licensed Practical Nurse (LPN) – Long Term Care
Location: New Berlin, NY 13411
Shift Schedule: 8-Hour Mid Shift Positions Available
Duration: 13 Weeks (Possible extension depending on business needs)
Guaranteed Hours: 40 Hours/ Week
Pay Range: $30 - $42/hr on w2 (Based on experience and qualifications)

Responsibilities:
Provide direct nursing care to residents in a long-term care setting under the direction of an RN or physician.
Administer medications and treatments as prescribed, ensuring accurate documentation.
Monitor and assess residents' health status and report significant changes to appropriate personnel.
Assist residents with activities of daily living (ADLs) such as bathing, dressing, mobility, and hygiene.
Collaborate with interdisciplinary team members to develop and implement individualized care plans.
Maintain accurate and timely documentation in accordance with facility and regulatory guidelines.
Comply with infection control, safety protocols, and all facility policies.
Provide emotional support and compassionate care to residents and their families.

Skills/Experience Needed:
Minimum of 2 years of recent experience in a Long-Term Care (LTC) setting – Required
Strong clinical and assessment skills in geriatric care
Familiarity with electronic health records (EHR) systems
Effective communication and interpersonal skills
Ability to work independently and collaboratively in a fast-paced environment

Education:
Graduate of an accredited LPN Nursing Program – Required
High School Diploma or GED – Required

Certifications:
Active NY LPN License – Required
BLS AHA (Basic Life Support) Certification – Required
COVID-19 Vaccination – Required per facility policy (Medical and religious exemptions accepted with valid documentation).
